All driving volunteers for sports and activities must complete this form and provide the school office with your driver’s license and most recent proof of insurance. The office staff will make a copy and return them to you. Privately owned vehicles may be used for transportation to certain school activities.
Parents who must provide transportation to and from school because free transportation is not available for their child may be able to receive money from the state to help offset some of the cost. In 2002, the Board of Education adopted a revised policy on medications in schools. The purpose of this policy is to comply with revised Illinois guidelines for medication administration, to protect all children from unsafe drug practices, and to provide a drug free environment. It is not the intention of this policy to make it difficult for children to receive necessary medication while at school. This policy applies to both normal school hours, as well as any extra-curricular activities that your child maybe participating in.
It is the players/parents responsibility to make arrangements for rides to and from an event. If a player will be riding with another parent, this form MUST be returned by the morning of the event. No player will be released unless it is turned in.
