Here are the Extracurricular Activities/Athletics Buses Procedure and the Google Form.   

All team members will ride the bus from school. If you wish to take your child home with you (or someone you designate) rather than riding on the bus home, we ask that you submit this Google form at least 24 hours before the game. If you aren't sure who will be driving your student home, you may place multiple names on the form.
